Refund Policy, Waiver and Disclosures: 

• No Refund Policy
• Once Payment is made, you concent to the following below.
•  I, the undersigned and paying parent or legal guardian of the registered child, do hereby consent to emergency medical treatment, or other medical treatment as may be necessary for the welfare of the registered child by a physician, qualified nurse, and/or hospital, in the event of injury or illness during all periods the paid/registered child is participating in the Flag Team Camp. 
• I hereby waive on behalf of myself and the paid/registered child any liability or claim against Musa Athletic Performance, Citrus College. 
• I know of no medical condition that the above-named child has that would preclude him/her from practicing/playing in this program. 
• I understand that pictures and video of the camp will be taken for promotional purposes. 
• I hereby give my consent for the paid child to participate in the Flag Team Camp. 
• Any programs and/or events described herein are not endorsed and will not be supervised by Citrus College
